Carer’s Passport and Statement digital tool

Digital tool to help you reflect on and document your responsibilities as a carer.

The confidential online questionnaire helps you consider the impact on your employment and the conversations you have with health, social care and other professionals who may be supporting you.

Carer's Passport

The Carer’s passport is part of the Civil Service’s workplace adjustments, and our tool prepares you for this. We do not write them.

By completing the confidential questionnaire, you’ll get an emailed a link to a record of your answers. This can be used to support you completing the Civil Service Carer’s Passport and to lead discussions with your line manager about workplace adjustments. You’ll be able to share this information with new managers.

You’ll also get a Carer’s Statement, if you requested one, and an Action Plan with tailored advice and signposts to additional resources.

Note: The Civil Service Carer’s Passport is not applicable for staff in the NICS. For further information on support for carers, please contact the NICS Welfare Department.

Note: A Carer’s Passport does not apply to childcare responsibilities unless your child has a long-term health or disability.

Carer's Statement

If you are a serving, former or retired civil servant or an unpaid carer of a civil servant, use our Carer’s Statement to help you communicate the impact of the role on your life with health and social care professionals.

By completing the questionnaire, you’ll get an emailed link to a record of your answers.

You’ll also get a Carer’s Passport, if you requested one, and an Action Plan with tailored advice and signposts to additional resources.

Note: A Carer’s Statement does not apply to childcare responsibilities unless your child has a long-term health or disability.

What do I need to know to get started?

We’ll ask you about your caring responsibilities and your wider situation so your document(s) can be completed as accurately as possible. This will enable us to provide advice and recommendations tailored to your personal circumstances.

Note: We don’t store your completed document(s) or Action Plan. You will receive them via email. You’ll need to download and save the documents as the link will expire after 28 days. If you have not received your emailed link within 24 hours, please email us at help@cfcs.org.uk

  • The questionnaire usually takes about ten minutes to complete. This will depend on the level of detail you provide.
  • You must complete your application in one session – you cannot save your answers and come back later.
  • You’ll be able to preview your document(s) and edit your answers during your session.
  • If your caring role changes in any way, you will need to complete a new application.
  • Please make sure to enter your email address correctly, as we will send your completed document(s) electronically.
